This microcredential is the first section in the Chronic Disease Management in the Community series. The purpose of this microcredential is to explore the basic concepts of chronic disease in community settings. Utilising the knowledge of chronic disease epidemiology, environmental factors’ impact, and the human experience of disease in assessment practice. It aims to integrate those basic concepts and principles in assessing people with chronic disease and complex care needs.
This microcredential is the first in a series of three that corresponds to the unit of study 339 Chronic Disease Management in the Community.
